Doctor rates black coffee combinations: Why cinnamon scores 9/10, milk gets 8.5 and ghee doesn't make the cut

Imagine your morning coffee lined up for a health report card. One cup is plain and bitter, another has a spoonful of sugar, a third gets a splash of milk, while others arrive with cinnamon, lemon or ghee. Each promises a different experience, but which one actually deserves the highest score? In a social media video, gastroenterologist Dr Shubham Vatsya rates six black coffee combinations, from plain black coffee to ghee and lemon. His verdict challenges the idea that adding a trendy ingredient automatically makes your cup healthier. Cinnamon scores 9/10, milk gets 8.5, and ghee receives 6. But the most interesting question is not which combination wins. It is what these ratings mean for your calories, nutrients, digestion and overall health.

1. Black coffee: 10/10

Dr Vatsya gives plain black coffee the top score, highlighting its low calorie content and natural caffeine. However, caffeine affects people differently. Too much can cause anxiety, palpitations, restlessness or sleep disruption.

2. Black coffee with sugar: 4/10

Coffee itself may be low in calories, but a spoonful of sugar changes its nutritional profile.Dr Vatsya gives this combination 4/10, pointing to the unnecessary addition of sugar. Sugar also raises blood glucose, triggering a normal insulin response; the size of that response depends on several factors, including the amount consumed and an individual’s metabolic health. It is too simplistic to treat every addition of sugar as a harmful insulin spike.

3. Black coffee with milk: 8.5/10

Milk changes more than the taste and colour of coffee. It contributes protein, calcium and other nutrients, while making the drink more filling. The calorie count will depend on the amount of milk and whether it is whole, low-fat or skimmed.Dr Vatsya rates this combination 8.5/10, while noting that lactose intolerance can cause bloating or discomfort in some people.

4. Black coffee with ghee: 6/10

Dr Vatsya gives it 6/10, arguing that the benefits are sometimes overstated by influencers.Ghee can make coffee richer and creamier, but it also adds concentrated calories and saturated fat.

5. Black coffee with cinnamon: 9/10

Cinnamon adds warmth and sweetness to coffee without requiring sugar. Dr Vatsya gives the combination 9/10 but warns against treating cinnamon as a fat-burning ingredient or a replacement for diabetes medication.Research on cinnamon and blood glucose has produced mixed results. Some studies suggest possible modest effects on blood sugar measurements, but the evidence is not strong enough to recommend cinnamon as a treatment for diabetes. Results from supplements also cannot automatically be applied to a small sprinkle in coffee.

6. Black coffee with lemon: 8/10

Lemon adds acidity and some vitamin C, although the amount provided by a small squeeze may be modest. Dr Vatsya rates this combination 8/10, citing vitamin C and antioxidants, while acknowledging that it has no proven detox advantage.Cleveland Clinic experts have also debunked the popular claim that lemon coffee burns fat. Lemon does not have a special mechanism for melting body fat, and its acidity may aggravate heartburn in people prone to acid reflux.

So, which black coffee combination is healthiest?

Dr Vatsya’s ratings favour plain black coffee for its low calorie content, cinnamon for flavour without much added sugar, and milk for its nutritional contribution. The best choice depends on what you want from your cup. If your goal is to limit calories, plain coffee or cinnamon may suit you. If you want more protein and calcium, milk adds useful nutrients. If you have acid reflux or are sensitive to caffeine, your own symptoms matter more than any online score.A healthier coffee habit does not require an exotic ingredient. Often, the simplest change is to choose additions you enjoy, keep added sugar modest and pay attention to how caffeine affects your sleep and comfort.
